International OMS Certification (IBCSOMS)

The International Board for the Certification of Specialists in Oral and Maxillofacial Surgery (IBCSOMS) is specific to oral and maxillofacial surgeons and was founded to define an international standard for specialists in oral and maxillofacial surgery. Candidates must successfully complete the certification process to attain initial Fellowship of the IBCSOMS and hold the credential of FIBCSOMS, which will be subject to a formal process of periodic re-evaluation.

Regulaciones

En varios países la cirugía oral y maxilofacial es una especialidad reconocida por una asociación profesional, como es el caso de la Asociación Dental Americana, el Colegio Real de Cirujanos de Inglaterra, el Colegio Real de Cirujanos de Edimburgo, el Colegio Real de Dentistas de Canadá, el Royal Australasian College De Cirujanos Dentales y el Consejo Federal de Odontología de Brasil (CFO).

En otros países, la cirugía oral y maxilofacial como especialidad existe, pero bajo diferentes formas, ya que el trabajo es a veces realizado por un solo o doble especialista calificado en función de las regulaciones de cada país y las oportunidades de formación disponibles.

Resumen

Un cirujano oral y maxilofacial es un cirujano especialista regional que trata todo el complejo craniomaxilofacial: área anatómica de la boca, mandíbulas, cara, cráneo, así como estructuras asociadas.

Dependiendo de la jurisdicción, los cirujanos maxilofaciales pueden requerir capacitación en odontología, cirugía y medicina general; La formación y la cualificación en medicina se pueden realizar opcionalmente incluso si no se requiere.

La cirugía oral y maxilofacial es ampliamente reconocida como una de las especialidades de la odontología. En el Reino Unido sin embargo, la cirugía maxilofacial es una especialidad médica que requiere tanto grados médicos como dentales, culminando en una calificación apropiada (por ejemplo, miembro del Royal College of Surgeons, FRCS, en el Reino Unido). Sin embargo, todos los cirujanos orales y maxilofaciales deben obtener un título universitario en odontología antes de comenzar el entrenamiento de residencia en cirugía oral y maxilofacial.

También pueden optar por seguir recibiendo capacitación en una formación de beca de cirugía oral y maxilofacial de 1 o 2 años en las siguientes áreas:

* Reconstrucción microvascular de cáncer de cabeza y cuello

* Cirugía facial cosmética

* Cirugía craneofacial / Cirugía maxilofacial pediátrica / Cirugía de hendidura

*Traumatismo craneo maxilofacial

*Reconstrucción de la cabeza y el cuello_ cirugía plástica de la región de cabeza y cuello

*Regeneración maxilofacial: reforma de la región facial mediante técnicas avanzadas de células madre

La popularidad de la cirugía oral y maxilofacial como una carrera para las personas cuyo primer grado fue medicina, no odontología, parece estar aumentando en unos pocos países de la UE [ aclaración necesaria ] . Sin embargo, los fondos públicos dedicados a 14 años de capacitación son una preocupación importante para los gobiernos. Los programas integrados son cada vez más disponibles para los graduados de medicina que les permite completar el requisito de grado dental en aproximadamente 3 años para que puedan avanzar para completar posteriormente la formación quirúrgica oral y maxilofacial.

Procedimientos quirúrgicos

Los tratamientos se pueden realizar en el complejo craniomaxilofacial: boca, mandíbulas, cara, cuello, cráneo e incluyen:

*Cirugía Dentoalveolar (cirugía para extraer los dientes impactados, extracciones de dientes difíciles, extracciones en pacientes médicamente comprometidos, injertos óseos o cirugía preprotésica para proporcionar una mejor anatomía para la colocación de implantes, dentaduras postizas u otras prótesis dentales)

*Cirugía para insertar implantes dentales osteointegrados (hueso fundido) e implantes maxilofaciales para fijar prótesis craneofaciales y audífonos anclados en hueso.

*Cirugía estética de la cabeza y el cuello: (ritidectomía / estiramiento facial, blefaroplastia / blefaroplastia asiática, otoplastia, rinoplastia, septoplastia, aumento de las mejillas, aumento del mentón, genioplastia, oculoplásticos, liposucción del cuello, realce labial, tratamientos cosméticos inyectables, botox, peeling químico Etc)

*Cirugía correctiva de mandíbula (cirugía ortognática), tratamiento quirúrgico y / o bloqueo de apnea del sueño, avance maxilomandibular, genioplastia

*Diagnóstico y tratamiento de:

o Patología benigna (quistes, tumores, etc.)

o Patología maligna (cáncer oral y de cabeza y cuello) con (cirugía ablativa y reconstructiva, microcirugía)

o Neoplasia cutánea (cáncer de piel), reconstrucción labial

o Malformaciones craneofaciales congénitas como labio leporino y paladar y malformaciones de la bóveda craneal como craneosinostosis, (cirugía craneofacial)

o Trastornos crónicos del dolor facial

o Trastornos de la articulación temporomandibular (ATM)

o Disgnatia (mordedura incorrecta) y cirugía reconstructiva ortognática (literalmente "mordida recta"), cirugía ortognática, avance maxilomandibular, corrección quirúrgica de la asimetría facial.

o Traumatismo de tejidos blandos y duros de la región oral y maxilofacial (fracturas de mandíbula, fracturas de huesos de la mejilla, fracturas nasales, fractura de LeFort, fracturas de cráneo y fracturas de ojo).

En el Reino Unido y la mayor parte de Europa

Oral y Maxilofacial (OMF) Los cirujanos se especializan en el diagnóstico y tratamiento de enfermedades que afectan la boca, las mandíbulas, la cara y el cuello.

La cirugía OMF es única en la exigencia de una doble titulación en medicina y odontología, y se suele considerar como el puente entre la medicina y la odontología, tratando condiciones que requieren experiencia de ambos antecedentes como cáncer de cabeza y cuello, enfermedades de las glándulas salivales, desproporción facial, dolor facial , Trastornos de la articulación temporomandibular, dientes impactados, quistes y tumores de las mandíbulas, así como numerosos problemas que afectan a la mucosa oral tales como úlceras bucales e infecciones. Muchos cirujanos OMF se centran en una de estas áreas para desarrollar un interés sub-especialista en el ámbito de la especialidad más amplia.

En Australia, Nueva Zelanda, América del Norte

La cirugía oral y maxilofacial es una de las nueve especialidades dentales reconocidas por la Asociación Dental Americana, el Colegio Real de Dentistas de Canadá, el Royal Australasian College of Dental Surgeons. La cirugía oral y maxilofacial requiere 3-6 años de más formación universitaria formal después de la escuela dental (DDS, BDent, DMD o BDS). En los Estados Unidos, los programas de cuatro años de residencia otorgan un certificado de especialidad en cirugía oral y maxilofacial. Los programas de residencia de seis años otorgan el certificado de especialidad además de un título como un título médico (MD, DO, MBBS, MBChB, etc.) o un título de investigación (MS, MSc, MPhil, MDS, MSD, MDSc, DClinDent, DSc , DMSc, o PhD). Tanto los graduados de 4 y 6 años son designados como "elegibles por la Junta Directiva de los Estados Unidos", los que obtienen "Certificación" son Diplomáticos. Aproximadamente el 50% de los programas de capacitación en los Estados Unidos y el 20% de los programas de capacitación canadienses son de "doble titulación".

El programa de entrenamiento típico para un cirujano oral y maxilofacial es:

*2 - 4 años de pregrado (BS, BA o grados equivalentes)

* 4 años de estudio dental (DMD, BDent, DDS o BDS)

*3 - 6 años de residencia de formación - Algunos programas integran un grado adicional como: una maestría (MS, MDS, MSc, MClinDent, MScDent, MDent), doctorado (PhD, DMSc, DClinDent, DSc) , MD, DO, MBChB, MDCM)

*Después de completar el entrenamiento quirúrgico, la mayoría de los exámenes de especialidad final son: (US "Board Certified (ABOMS)"), (Australia / NZ: "FRACDS") o (Canadá: "FRCDC"

*Algunos Colegios ofrecen Membresía o Becas en cirugía oral / maxilofacial: MOralSurg RCS, M (OMS) RCPS, FFD RCSI, FEBOS, FACOMS, FFD RCS, FAMS, FCDSHK, FCMFOS (SA)

*Muchos cirujanos orales y maxilofaciales de doble cualificación ahora también están obteniendo becas con el Colegio Americano de Cirujanos (FACS por sus siglas en inglés)

*Duración total media después de la escuela secundaria: 12 - 14 años

 Además, los graduados de los programas de entrenamiento de cirugía oral y maxilofacial pueden perseguir becas, generalmente de 1 a 2 años de duración, en las siguientes áreas:

* Cáncer de cabeza y cuello: reconstrucción microvascular

A special program in Nijmegen offers a shorter possibility, i.e.. if you start with medicine, you can study dentistry for 2 full-time years. This program is called TOVA (Tandheelkundige opleidig voor Artsen –Dentistry for Medical Doctors): Total of 8 years training.

Based upon the The Bologna Declaration on the European space for higher education, the Dutch government has reduced the amount of students that follow a second university or academic study as undergraduate student after finishing an academic study with a diploma. Dutch government will only provide one Bachelors and one Masters degree. Costs related to a second Bachelors or Masters degree must be paid by the individual candidate him- or herself. Consequently there is still a debate in the Ministry of Health about the regulations concerning the mandatory dual degree training program of Oral and Maxillofacial Surgery. Further, there is also debate in the Ministry of Health about the position of the Dutch dual degreed Oral and Maxillofacial Surgeons in the European Community. In the Netherlands, Oral and Maxillofacial Surgery is a Dental Specialty, but on a European level it should be a Medical Specialty because of the regulations in the European Community (dual degree means a medical doctor with an additional degree, i.e. dentistry). In summary: still the same problems as in the last years.

In their first year the ‘Basic Surgical Examination’ that is organised by the ‘Dutch College of Surgical Specialties’ and which is required for all surgical specialties, has to be passed. In the next 3 years, residents have to follow twice yearly a two-day course dealing with all various topics of the specialty, followed by an examination, which has to be passed successfully. Recently the association also introduced the application of the Entrustable Professional Activities (EPA’s), a framework for teaching and assessing competencies which is intended to start 1-1-2020 . 2 years of postgraduate study in general dentistry (internship)

6 months of special training in Surgical Stomatology

There are also possibilities to study at residentship (2 years) and aspirantship (4 years) that are not obligatory for practical medicine. But they are recommended for specialists working in sphere of science and education (PhD Diploma)

The prerequisite of entry is holding both an Medical and registrable dental degree. Dental courses for qualified doctors vary from 3 to 5 years, as do Medical course of qualified dentists. The 18 years represents an average length of time spent from entry to Dental school (currently the most common first degree) to finishing higher training.
